Teen facing homicide by vehicle charges involving 2024 crash

Zakary Kluchinsky, 18, of Girardville, is facing charges of homicide by motor vehicle, aggravated assault by vehicle and driving under the influence as a result of the investigation of a motor vehicle crash that happened at 4:21 a.m. July 31 last year in West Mahanoy Township.

State police at the Frackville barracks announced he was taken into custody, arraigned before Magisterial District Judge Anthony J. Kilker and committed to the Schuylkill County Prison, Pottsville, in lieu of $100,000 bail.

Police said the incident happened on the Gold Star Highway in the township. Police said Jenna M. Sallade, 18, of Mahanoy City, died in the crash involving a 1998 Honda Civic.

At the time, police said the other occupants of the vehicle were Kluchinsky, and Jeffrey E. Livergood, 22, and Logan Payne, 18, both of Frackville.

Police said Livergood and Payne had serious injuries and were transported by personnel from the Shenandoah Community Ambulance Association and the Pottsville/Schuylkill Haven Area EMS unit to the Geisinger Medical Center, Danville.

Kluchuinsky was taken to the Lehigh Valley Hospital, Salisbury Township, by members of the Minersville EMS unit.

Police said the vehicle was southbound and struck an embankment off the west side of the road, rolled over several

times, causing all four occupants to be ejected from the car, which came to rest on the median of the road facing east.
