A Mahanoy City teen died, and three other people were injured, in a one-vehicle crash at 4:21 a.m. July 31 on the Gold Star Highway (Route 924) in West Mahanoy Township.

State police at the Frackville barracks said Jenna M. Sallade, 18, was pronounced dead at the scene.

The other occupants of 1998 Honda Civic were Zakary T. Kluchinsky, 18, of Frackville, Jeffrey E. Livergood, 22, and Logan Payne, 18, both also of Frackville.

Police said Livergood and Payne had serious injuries and were transported by personnel from the Shenandoah Community Ambulance Association and the Pottsville/Schuylkill Haven Area EMS unit to the Geisinger Medical Center, Danville.

The severity of Kluchinsky’s injuries is unknown, police said. He was taken to the Lehigh Valley Hospital, Salisbury Township, by members of the Minersville EMS unit.

Police said the vehicle was southbound and struck an embankment off the west side of the road, rolled over several times, causing all four occupants to be ejected from the car, which came to rest on the median of the road facing east.

Police did not say who was driving.
