Job description: My job is working with patients to ensure that they have what is needed for them to successfully discharge from the hospital. This can include home health care, rehab placements, medical equipment, and community based services. I like being part of a team that cares about our community.

Challenges/proud moments of your job: Assisting a patient or their loved ones in making difficult decisions, is painful for everyone involved. Helping them through that process is meaningful to me.

Background/Education: Bachelor degree from Kutztown University

Family information: I live with my husband Blaine and our son, Gavin. I enjoy spending time with my family and friends, and traveling.
